When the PC forgot something that could have saved them?

OK, lets say you have a game that goes pear shaped for a PC or three.
A while down the road, one of those PCs remembered an item, spell or some other bonus (or item) that would have spelled the difference between life and death for that PC.

Do you retcon it so he/she is now back alive? Do you tell them "tough cookies"?
Does it matter if its only say a few hours IN GAME since that death occurred, vice say a few DAYS IRL since it happened?

Tough cookies if it's been longer than maybe a half hour irl otherwise maybe figure some way to bring them back
